RESIDENTIAL HABILITATION SERVICES & SUPPORTS

Personalized care in a sTABLE home environment

Our Residential Habilitation Services and Supports (RHSS) programs are designed to ensure the health, safety and welfare of individuals with developmental disabilities. Digni-Health facilitates the connection between IDD and staff or family caregivers capable of meeting their needs while providing a range of placement options with customized levels of supervision and independence based on the individual's strengths and requirements.

INDIVIDUAL services (irss)

Our individual residential services through Host Homes provide a safe living environment with tailored programing. Our family caregivers are prepared to invite individuals with intellectual or developmental disabilities into their homes, offering them the opportunity to receive care within a familial environment. These individuals will benefit from companionship, support with daily tasks, involvement in family events, and integration into the community.

GROUP services (GRSS)

Digni-Health's PCA Homes encompass a safe & comfortable group living environment of four (4) to eight (8) individuals with intellectual or developmental disabilities. We offer group residential settings with dedicated staff experienced and qualified in assisting IDD with daily living activities such as personal hygiene, grocery shopping, budgeting, toileting, feeding, and bathing, as well as companionship and integration into the community.

Digni-Health LLC is seeking compassionate Direct Support Professionals (DSPs) to join our team. DSPs are at the frontline of supporting individuals with developmental disabilities, empowering them to lead fulfilling and independent lives within our communities.

Responsibilities may include fostering relationships and community connections, teaching essential life skills such as financial management, appropriate social behaviors, accessing transportation, self-care routines (e.g., grooming, toileting, eating, dressing), communication techniques, socialization activities, safety practices, positive behavior reinforcement, vocational training, domestic skills, and other specialized areas as needed.
